What Happens When You Come in with a Toothache

Dr. Bhatia starts by asking about your symptoms: where the pain is, when it started, what makes it better or worse, and whether it is constant or comes and goes. She then examines the tooth visually, tests it with cold and pressure, and takes digital x-rays to see what is happening inside the tooth and bone.

Within minutes, she has a diagnosis. She will show you the x-ray on the screen, explain what is causing the pain in plain terms, and walk you through your treatment options. If a filling can fix it, she does it right then. If a root canal is needed, she can start the same day. If the tooth cannot be saved, she will explain the extraction process and cost and discuss replacement options.

If you are anxious on top of being in pain, nitrous oxide is available immediately. The goal is to get you out of pain and address the underlying problem before you leave, not send you home with painkillers and a follow-up appointment in two weeks.

What causes a toothache?

The most common causes are cavities (tooth decay that has reached the nerve), cracked or fractured teeth, dental abscesses (infections at the root of the tooth), gum disease, exposed tooth roots from receding gums, and a damaged or lost filling. Less common causes include sinus infections, teeth grinding, and TMJ issues. Dr. Bhatia will take x-rays and do a thorough exam to determine the exact cause.

What should I do while waiting to see the dentist?

Take over-the-counter ibuprofen (Advil/Motrin) for pain and inflammation, following the dosage instructions on the bottle. Avoid very hot, cold, or sweet foods and drinks on the affected side. If there is swelling, apply a cold compress to the outside of your cheek for 15 minutes on, 15 minutes off. Do not put aspirin directly on the tooth or gums. When is a toothache a dental emergency?

Seek same-day care if you have severe pain that does not respond to over-the-counter medication, swelling in your face, jaw, or neck, a fever along with tooth pain, a knocked-out or badly broken tooth, or bleeding that will not stop. These can indicate a serious infection or injury that needs prompt treatment. Call us immediately or walk in during office hours.

Can a toothache go away on its own?

Sometimes tooth pain temporarily subsides, but that does not mean the problem is gone. If a toothache goes away suddenly after being severe, it may mean the nerve inside the tooth has died, which usually leads to an abscess. If pain comes and goes, it typically means the underlying issue (cavity, crack, infection) is progressing. The safest approach is to get it evaluated regardless of whether the pain improves.

Will I need a root canal for my toothache?

Not necessarily. Many toothaches are caused by cavities that can be treated with a filling. Root canals are needed when the decay or damage has reached the nerve (pulp) inside the tooth. Dr. Bhatia will take x-rays and test the tooth to determine whether a filling, root canal, extraction, or other treatment is appropriate. She will explain all your options before starting anything.

How much does it cost to treat a toothache?

The cost depends on what is causing the pain. A filling might be $150 to $400. A root canal ranges from $700 to $1,500. An extraction is $150 to $600. An emergency exam with x-rays is the starting point. Most insurance plans cover emergency exams and medically necessary treatment. We verify your benefits and give you a cost estimate before starting any work.

Can I just get antibiotics for my toothache?

Antibiotics can help if there is a bacterial infection (abscess), but they are not a standalone treatment for tooth pain. Antibiotics reduce the infection temporarily, but without treating the underlying cause (removing the decay, doing a root canal, or extracting the tooth), the infection will return. Dr. Bhatia prescribes antibiotics when appropriate but always pairs them with a plan to address the root cause.
